Employing a patient and family centered approach, this group’s purpose is to develop best practice guidelines and tools to ensure an optimal transition/transfer of care to adult services for patients with NDD. This task force also strives to improve the process for the clinicians and their staff on both the pediatric and adult care side by providing structure to safely and effectively integrate patients into adult practices.
