Recording Notice: Please Read
All sessions of the Pediatric Stroke Champions live workshop will be recorded. Recordings will include audio, video, and the chat transcript. If you do not wish to have your voice or appearance recorded, you may choose to keep your camera off and microphone muted, and to interact via the chat function.
How Will the Recordings Be Used?
1. Recordings will be posted to the course website for 6 weeks following the live session so that registered attendees may review the presentations on-demand. Given the highly interactive nature of the course, however, we strongly encourage live attendance for the best learning experience.
2. In addition to posting the recordings to this course site, conference recordings may be sectioned into derivative courses for Boston Children’s Hospital’s
On-demand Continuing Education offerings, which may be posted indefinitely.
Authorization to Use Recordings in On-Demand Courses
The following release is necessary for Boston Children’s Hospital to include your recording in enduring online continuing education materials. Please indicate your preference with the options below. If you have any questions, please contact the CME department at cmedepartment@childrens.harvard.edu

I authorize Boston Children’s Hospital to make, reproduce, publish, distribute, and otherwise use for all purposes related to Continuing Education photographs, videos, and/or sound recordings of me. I further consent to and authorize the use of such materials in any electronic distribution, through Continuing Education, in whatever manner Boston Children’s Hospital determines in its sole discretion. I expressly waive any approval rights I may have with respect to such materials. I agree that I will not be paid now or in the future for use of my image or recording. I am aware that Boston Children’s Hospital cannot control how the recipients of Continuing Education training use or share the information, and that laws protecting its confidentiality at Boston Children’s Hospital may or may not protect this information once it has been incorporated within the Continuing Education project.
I may cancel this authorization in writing at any time, except in cases where Continuing Education has already released an image or video. All revocations must be sent in writing to: Boston Children’s Hospital, Marketing and Communications Department, 300 Longwood Avenue – LM 6168, Boston, MA 02115.
I hereby release and discharge Boston Children’s Hospital, its officers, agents, representatives, successors, and assigns and all persons acting under their permission or authority from all liability of any nature arising from my participation in the Continuing Education project. I expressly release Children’s from any liability for any violation of any personal or property rights relating to the use of my likeness or personal information that may arise out of the publication, duplication, broadcast or exhibition of our involvement with Continuing Education.
I UNDERSTAND THAT THE CARE PROVIDED BY BOSTON CHILDREN’S HOSPITAL WILL NOT BE AFFECTED IF I DO NOT AUTHORIZE THIS RELEASE.
